Action Plans

An Asthma Action Plan must be filled out by your child's medical doctor if your child has asthma. 

A Food Allergy Action Plan must be filled out by your child's medical doctor if your child has food allergies. 

A Diabetes Medical Management Plan must be filled out by your child's medical doctor if your child has diabetes. 

A Seizure Action Plan must be filled out by your child's medical doctor if your child has epilepsy/seizures. 

You can email the School Nurse for the Action Plans.  Once completed, please return these forms to the School Nurse. 

The forms must be renewed annually.

The Self-Administration form and Medication Administration form must also be filled out if the student is to carry their asthma inhaler and self-administer during school hours.
